Possessing an 'About Me' card does not guarantee that all patient preferences will be met.

Clinicians and staff will use the card as a tool to initiate dialogue and will make every effort to accommodate the patient’s needs, within the limits of clinical and operational constraints.

Here are some examples of what we can support you with and some we are unable to support you with.

This card is currently only recognised at Warrington Hospital.

  • Chaperone/Same sex clinician on request
  • No windowless room
  • Sit next to a door
  • Loud noises (limit)
  • Interpretation required
  • Time to process what is happening/information given
  • Speak in a language I understand free from medical terms
  • Include me and my carer in the conversation (don’t talk about me talk to me)
  • Wait in adjacent area/outside area/in car
  • Bright lights (reduce)
  • Extend appointment time
  • Honest conversation, wait time, process
No Reasonable adjustments
  • Use the card to be seen quicker
  • Extra accommodations that don't benefit he patient/ professional or improve service
  • cannot be used as a sick note for work/ benefits
  • Skipping Medical Protocols
  • Refusing to Follow Infection Control Measures
  • Requesting Non-Evidence-Based Treatments
  • Avoiding Legal or Safeguarding Procedures
  • Requesting Home Visits Without Medical Need
